Vital Statistics
The Vital Statistics Division keeps birth and death records back to the year 1908. You no longer have to go to the county where you were born in order to get your birth certificate. You can obtain a certified copy of your birth certificate anywhere in the state of Ohio.

Death certificates can only be obtained in the county where the death took place.

Method of payment is cash, local check, or credit card.

You can obtain a certificate in person by coming to the Health Department office at 317 S. Fayette Street, Washington Court House, OH 43160 between the hours of 8 a.m. and 4:30 p.m.

A request can be sent in the mail with the person's name, date of birth/death, self addressed stamped envelope and a money order for $25.00.

We can also take requests over the phone. Phone orders will need to be paid by credit card only.
