The Vital Statistics Division keeps birth and death records back to the year 1908. The office
provides certified birth certificates, which are used for passports, age verification, SSI benefits,
genealogy studies or insurance.
Certified copies are available for $22 per copy for birth and death certificates.
Birth certificates are available for anyone born in Ohio, back to 1908. Death certificates are
available only for Fayette County, back to 1908.
Requests can be sent by mail. Send a note with the name and date of the birth or death, a money
order for $22, and a self-addressed stamped envelope.
The Health Department also takes requests over the phone for births and deaths, with payment by
